Government emergency economic measures
The government will provide a uniform benefit of 100,000 yen to all persons listed in the Basic Resident Register as of April 27. The acceptance and start date of benefits are determined by the municipality. The details of the system are made by the Ministry of Internal Affairs and Communications.
Mailing application method: Fill in the transfer account in the application mailed from the municipality and mail it to the municipality with a copy of the transfer account confirmation document and identity verification document
Online application method (Available to holders of My Number Card): Enter the transfer account from the minor portal, upload the transfer account confirmation document, and apply electronically.

SAMPLE FORM with TRANSLATIONS
This form will be mailed to the head of household of every family by the municipality where you are currently registered estimatedly around mid May to June depending on the municipality you are in. The head of household must fill this up and submit it back to the municipality by mail or if you don't have a bank account , you can go directly to the window in the municipality who is in charged of this cash benefit. The deadline of submission will be 3 months after the date of application started.
