Basically, there are two types of payment procedures: "procedures based on the application form sent from the city" and "online application (only for my number card holders)", which we are preparing for the beginning of June.
Please apply only for those who need a special fixed benefit as soon as possible.

If you are using this download version, please download and print the application form and consent form below, fill in the necessary information, attach the confirmation document and mail it to the following destination. (Envelopes and shipping charges will be borne by the applicant. In order to prevent the spread of new coronavirus infection, we will not distribute it at the counter.)
* In the case of the basic "procedure based on the application form sent from the city", no shipping fee will be charged.

Even if you use this procedure, the application form may still be sent separately by mail from the city, but the benefit will only be paid once. Please note.
Also, if the information you have entered is incomplete, insufficient, or unclear, the payment date will be the same as the basic "procedure based on the application sent from the city". Please be careful not to make a mistake when filling in.
